the temple comes to them?
My PC's have a home in Hommet (long story). I am considering for if they take a long time out of the dungeon and take out two or so Temples for the folks of the Outer fane to attack their home.

There is an NPC that used to adventure with the group who lost a hand. She is not good, she also lost a brother there (he was "petrified" by an illusionary baskalik). I am considering she spilling her guts to the Inner fane guys and maybe trying something more direct. The main thing stopping me is many NPCs have turned on the group in the past and it seems done to death. I could also have her, quietly inform them to the group's strengths and weaknesses and do nothing open. What do you guys think? Looking for ideas, opinions.

Perhaps you can put a "positive" spin on the attack on the PC's home. This would be in constrast to everyone always working against them. Let's say the PC's get back to Hommlet to find their house charred slightly (one corner), but otherwise undamaged. They also find a lot of townspeople around it with buckets and a few dead townspeople, including the NPC.

The NPC could be the orchestrator of the defense. She got wind of the impending attack and got a change of heart at the last minute, in favor of the PC's who she now realizes have done nothing but help her. She convinces some townspeople (militia members perhaps) of the same thing and they help defend the home against the temple-paid bad guys. You can make them whoever fits in your campaign best (orcs perhaps, or human warriors, or even gnolls, whoever, with one leader who got away). The PC's might even go so far as to get the NPC raised and or regenerated. Might be some cool RP. It might also help to keep your players from feeling you are a mean DM. :)

On the other hand :) this might be a great opportunity to foreshadow the Infernal Device. Have the NPC return as part of the temple's attack force, augmented with a demonic arm where the missing hand used to be. IF they remove the demonic arm, and have an Atonement cast on the NPC, they'll discover that she didn't in fact turn on them, but rather was tortured for information before being cast into the Device, and only joined the Temple's forces after the arm drove her to the side of Evil.

No to backstabbing back home
Infiniti2000's suggestion is good.

I'd steer away from ordinary folks 'back home' turning out to be enemies, or 'taking away' their home base.

The PCs need somewhere 'safe'. They need to feel some sense of community, and feel that the community is worth defending - if it seems nobody in Hommlet cares about them or has any 'good' qualities, player interest in roleplaying and forming relationships with anyone outside the PC group can wane fast - much to the detriment of the campaign, and everyone's enjoyment.

...and don't kill their pets or families either :evil

Thanks for the ideas. I am going to go with an amalgamation of them. I am going to have Belinda (the NPC) spill the beans on the group's home, have a demonic appendage put on to replace the hand (and then some) have her lead the charge to stop the sacking of their home but not killed (change of heart). The place will be singed and the group will be happy the townsfolk helped save their home. Belinda will have ratted them out in exchange for her hand to be fixed (and maybe get her brother's fate) but will not tell the truth. This will accomplish a few things: make the PC's feel their good deeds have been rewarded and noticed, advance a subplot with Belinda, foreshadow the Infernal Device and Belinda's subsequent wisdom losses will be a drain on their abundant resources and help set the tone for the adventure.

I thought you said that the NPC was "not good"? Only good characters are driven insane by the Device; neutral characters simply give in to evil temptation (and eventually become evil) when they fail their saves.
