I'm doing one more check of all the conversions I've made here before I declare them officially done. Now that WoTC has finally released their conversion guide, I've already noticed a couple of issues.
8/9: Added acid splash, message, touch of fatigue to the spellbooks of all the wizards.
8/9: Added a second prohibited school to Nierethi and adjusted his spellbook to account for this.

Sorry to be nitpicky, but as far as I can tell Garrik's flat-footed AC should be 15 (his +1 Dex modifier is lost), not 16.

Xaod's feat change

I think making a call like this is outside of the scope of the conversion - our basic rule is convert with as few changes as possible... Combat Reflexes still allows you to make an AoO when flat-footed, so there is some benefit to Xaod. I think the note about the feat not doing much for Xaod belongs in the notes afterwards.

I disagree. I believe that this is an error, given that Xaod gains no significant benefit from the feat. (I don't think that flat-footed AoO's are worth a feat. I think that Monte likely overlooked Xaod's low DEX.) And correcting errors is very much within the scope of the conversion.
If you feel differently, then please feel free to add an alternate Xaod conversion.

I plugged his skill points into my spreadsheet, and come up with 102 spent. I believe he should have 104 total, leaving two unspent. (5th level rogue = 8x(8+3+1) = 96, 1st level assassin = 1x(4+3+1) = 8, 96+8=104)

I think it is more likely just that as an NPC Xaod doesn't have the carefully min-maxed feat selection that a PC would. It may have even been carefully selected by Monte not to provide much of a benefit... If Xaod ends up fighting alongside the PCs, this can help ensure he doesn't outshine them.
Most importantly the feat does do something, so I don't think we should be bringing personal judgments into it. That should be noted in the comments and left to the DM to decide.

I think it's a synergy bonus. 5 ranks of bluff grant a +2 to sleight of hand. 8 ranks + 2 dex + 2 synergy = +12.
Also, just a nitpick, but it's "sleight" of hand, not slight of hand.

Another Xaod suggestion
I would suggest adding an orange note that the value of the glove of storing has increased significantly in 3.5e, from 2500 (?) to 10000gp, in case DMs want to make a change to something more inline with his character level.

Phew!
Just looked through my part of the conversions and (hopefully) incorporated any errors you've mentioned so far.
Just to let you know I'm still here.
Oh, and I'm going to convert Big U soon.
And I noticed a minor typo in Toridans writeup. Fighter is abbreviated "Ftr" not "Fig".

Now that skeletons are templated, there is no appropriate skeleton for the encounter 25 of the moathouse. I made an ogre skeleton, which fit near the CR needed and fit the description in the text. I don't have access to my 3.0 things so I have no idea what's different and what's the same, however.
