Nierethi Poscurian, Champion of Elemental Evil
In my game, Lareth the Beautiful was captured by the Badgers and hung by Y'dey. Nierethi stole his corpse, and performed a ritual involving consuming Lareth’s brain to steal his destiny. This caused widespread elemental calamities, nearly destroying Hommlet, and was the 'radiation accident' that precipitated the switch to 3.5 in my game.
The 3.5 conversion switch Nierethi to Bard 4; for that purpose, I swapped his Int (14) and Cha (10) from the module. Since that time, he's gained 3 levels of Cleric (Tharizdun), and 3 levels of Mystic Theurge. Now that Hedrack has confirmed him as the Champion of Elemental Evil, he's picked up all 4 half-elemental templates, as suggested in the BOB. He hasn’t gained any levels since being confirmed as the Champion, so his skill points are calculated based on his Int of 10 before applying the half-elemental templates. For his 8th level, he bumped up his Con score; with the d4 hit dice from Mystic Theurge, the extra HP are quite welcome.
I’ve listed his CR as 14, which is a rough estimate of the actual challenge he poses. The 3 Cleric levels are treated as non-associated to his higher Bard level, so they only contribute +2 CR. Similarly, while the first half-elemental template adds +2 CR, each additional half-elemental template only adds +1 CR, since each template is made weaker by the fact that the other templates don’t grant any HD increase.

I like this. I would have left him an Illusionist, but that's really just me.
Couple of Quick Questions
1. Did the PCs encounter him in Hommlet before?
2. They obviously didn't kill him, so what did they think of him?
3. And how surprised were they to learn this?

Oh, yes, they encountered him. They broke into his house, stole everything of value, and then burned it down, all the while framing him for the arson (it was a rented room, and the landlord saw them enter). Then they got him drunk, pumped him for information, and beat him up and had him dragged out of town naked by an angry wolf. The party rog/wiz stole his spellbooks, but those subsequently disappeared when we switched to 3.5.
Suffice it to say, Nierethi has a bone to pick with them.
Since then, they've encountered him robed and disguised, in the ghoul warrens under the moathouse, when they came back to destroy the altar there (after figuring out what it did in the Fire temple). He had been holed up there after the ritual, where he was gaining his levels as a Cleric of Tharizdun. He escaped, and the party doesn't know who it was. In fact, they initially thought it was actually Lareth, raised from the dead, but I told them that the robed figure was much too short to be Lareth.
